About the Role:
We're seeking a compassionate and skilled Medical Assistant - Healthguide to join our team. In this role, you'll play a vital part in bridging the gap between patients and their primary care providers. Your mission will be to support patients in navigating their healthcare journey, addressing their concerns, and enhancing their overall well-being.
Key Responsibilities:
Work remotely with patient medical practice staff and our Case Management team to manage a designated patient population.
Collaborate independently and as part of a team to assist patients in achieving improved health outcomes.
Address patient inquiries, connect them with their medical practice, and implement effective care plans.
Document patient medical histories and issues accurately in electronic health records (EHR).
Understand and facilitate the referral and prior authorization processes.
Build and maintain trusting relationships with medical practices and our organization.
Monitor and meet performance indicators to promote positive patient outcomes and cost-efficiency.
Use various communication methods (calls, texts, emails) to engage with high-risk patients.
Escalate medical issues appropriately to registered nurses, primary care physicians, or leadership.
Travel occasionally to support primary care physician offices.
Follow security protocols to protect all protected health information (PHI) and intellectual properties.
What You'll Need:
Certification or registration as a Medical Assistant.
Bilingual in English and Spanish (required); medical interpreter skills are a plus.
A passion for coordinating patient care and serving individuals with complex medical and social needs.
Empathy, proficiency in motivational interviewing, and strong behavioral change management skills.
Excellent communication skills and the ability to document patient interactions accurately.
Familiarity with medical terminology and primary care practice operations.
Knowledge of social determinants of health.
Certified Emergency Medical Technician (preferred).
Experience with EHR systems (e.g., EPIC, Athena, Cerner) and various software applications.
Ability to manage multiple tasks effectively while maintaining attention to detail.
Demonstrated success in a remote work environment.

Compensation:
The hourly rate for this role ranges from $20 to $23. The final pay rate will be determined based on factors such as experience, education, and location.
