About the Role:
As a Remote Care Coordinator, you will play a vital role in linking services and coordinating care for children, youth, and families in need. Your responsibilities will include:

Acting as a liaison care coordinator across designated counties.
Providing immediate assistance in counties requiring support.
Collaborating with families to identify care coordination needs that align with service delivery standards and desired outcomes.
Understanding cultural factors that influence family dynamics to enhance engagement and success in care planning.
Identifying the strengths of children and families to foster positive engagement and support healthy outcomes.
Coordinating family-based services across home, school, and community settings.
Ensuring that services outlined in care plans are appropriate, least restrictive, and address the safety and treatment needs of families.
Building positive relationships with children, youth, and families to support effective integration of care plans.
Developing partnerships with community resources to meet the diverse needs of families.
Maintaining documentation, participating in evaluations, attending meetings, and staying informed about community resources.
Minimum Requirements:
To be successful in this role, you should have:

Experience in supporting children and families connected to behavioral health or related human services fields:
Three years with a high school diploma or equivalent; or
Two years with an associate or bachelor's degree; or
One year with a master's degree or higher.
Knowledge and experience in High Fidelity Wraparound (training provided).
Preferred experience in coordinated supportive services or care coordination.
Experience working with individuals with autism spectrum disorders or developmental disabilities.
Familiarity with family systems, community resources, and case management.
A culturally responsive approach with training and experience in managing complex cases.
Strong organizational skills and the ability to prioritize multiple tasks.
Excellent communication skills, both verbal and written.
Reliable internet access and the ability to operate in an automated office environment.
A valid driver's license.

Eligible Counties:
This position requires residence in one of our service counties or a contiguous county, including Coshocton, Fairfield, Guernsey, Morgan, Muskingum, Noble, Perry, Washington, Athens, Fayette, Gallia, Jackson, Highland, Hocking, Meigs, Pickaway, Pike, Ross, or Vinton. Quarterly in-person meetings will be held in various counties.
