We are seeking a motivated and dedicated Independent Living Coordinator to join our team and support our mission.

Job Summary: The Independent Living Coordinator will be responsible for overseeing and coordinating services for individuals transitioning to independent living. This role involves developing personalized care plans, supervising caseloads, and ensuring that clients receive the support they need to thrive in their living environments. The ideal candidate will have experience in the field of developmental disabilities, excellent organizational skills, and a passion for helping others achieve their highest potential.

Key Responsibilities:

  • Develop and implement individualized care plans for clients transitioning to independent living.
  • Coordinate and supervise a team of personal attendants and support staff.
  • Conduct regular assessments to monitor client progress and adjust care plans as needed.
  • Facilitate communication between clients, families, staff, and other service providers.
  • Provide training and support to staff to ensure high-quality service delivery.
  • Organize and schedule quarterly, semi-annual, and annual meetings with service coordinators and families.
  • Ensure compliance with all relevant regulations and standards.
  • Maintain accurate and up-to-date documentation of client care and progress.
  • Assist clients with daily living activities and promote independence.
  • Address and resolve any issues or concerns related to client care.

Qualifications:

  • AA Degree or 2 years related field experience preferred.
  • Minimum of 2 years of experience in the field of developmental disabilities or a similar role.
  • Strong organizational and time management skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Knowledge of relevant regulations and best practices in the field.
  • Compassionate and dedicated to making a positive impact in the lives of others.
  • Valid driver’s license and reliable transportation.
