Overview:

Community Health Parters is seeking an Advance Practice Provider (APP) to work in an established Maternal Fetal Medicine practice. The APP will provide compassionate, personalized care for high-risk obstretric patients in collaboration with Maternal Fetal Medicine physicians.

Education

• Completion of a Nurse Practitioner or Physician Assistant program required
• Credentialed by the CMC Medical Staff Office as an Allied Health Professional and privileged to perform Nurse Practitioner or Physician Assistant Standardized Procedures required

Experience

• 2 years of clinical experience in the area of specialty preferred

Licenses and Certifications

• If qualifying as a Nurse Practitioner, the following is required

o NP - Current State of California Nurse Practitioner License required
o NPF - Nurse Practitioner Furnishing Number required
o If qualifying as a Physician Assistant, the following is required
o PA - State of California Physician Assistant License required
o DEA - DEA Controlled Substance Number required
o BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
o Ability to participate in a federally funded program (Medicare and Medicaid) required
o NPC - National Certified Nurse Practitioner by ANCC (American Nurses Credentialing Center) or AANP (American Academy of Nurse Practitioners) preferred
o CRNFA - Certified RN First Assistant preferred Disclaimers:
