**Job Description**
We are seeking a Full-time Care Coordinator to join our team in Wichita, KS. This role is vital in ensuring we provide exceptional care services. You'll work closely with clinicians, staff, and patients to achieve healthcare goals and maintain open lines of communication. The ideal candidate is compassionate, experienced, and highly organized.
**Responsibilities:**
- Develop and manage individualized patient care plans using our electronic medical record system.
- Maintain frequent contact with patients to provide support, coordinate care, and enhance positive outcomes.
- Accurately document all client communications, both verbal and written.
- Liaise with Primary Care Clinicians to address patient emergencies and ensure comprehensive care plans are complete and accurate.
- Keep the Team Supervisor informed of all relevant care plan issues and any known or perceived concerns.
- Collaborate effectively with various cross-organizational areas to meet patient, family, and partner facility needs.
- Proficiently use technology, including secure email, telephone systems, and electronic medical records.
- Adhere to documentation protocols and best practices for daily work logs, issue escalation, and internal communications.
- Demonstrate excellent customer service skills, evidenced by positive feedback from customers and patients.
- Contribute positively to the team by supporting all members in a productive and constructive manner.
- Operate office equipment such as computers, telephones, and copy machines as necessary.
**Qualifications:**
- Comfortable with extensive phone communication.
- Compassionate and empathetic, with exceptional customer service skills.
- Ability to take accurate notes and document tasks promptly.
- Strong multitasking skills, with the ability to manage multiple patients and workstreams efficiently.
- Thrive in a fast-paced environment.
- Available to work from 8:30 AM - 5:30 PM, Monday through Friday.
- Proficient in using computers, including Outlook and other Microsoft Office programs.
- Basic knowledge of healthcare terms, conditions, roles, and care principles is beneficial.
- Must pass a drug screen and background check, possess a positive attitude, adapt positively to change, be a team player, and be willing to learn new skills continuously.
