REGISTERED NURSE
Definition :
Provides skilled nursing care in accordance with the client's plan of treatment, including comprehensive health and psycho social evaluation, monitoring of services, teaching and training activities and provision of direct nursing care.
Level of Responsibility :
Works under the supervision of the Clinical Manager. Provides supervision of assigned personnel including licensed practical nurses, home health aides, and personal care providers.
Examples of Duties and Responsibilities :
Provides initial and ongoing comprehensive assessments of the client's needs, including OASIS assessments at appropriate times. Initiates the plan of care and revises as necessary.Teaches, supervises, and counsels the client and family members regarding the disease process, self-care techniques, prevention strategies, and other related problems of the client.
Provides those services requiring substantial and specialized nursing skill.
Initiates appropriate preventive and rehabilitative nursing procedures.
Interprets the care plan and demonstrates appropriate procedures as necessary to LPNs, aides, etc.
Maintains accurate records and reports including those required by Medicare and other third-party payers.
Informs the physician and other personnel of changes in the client's needs.
Evaluates the effectiveness and outcomes of care. Plans for the client's discharge from service.Serves as coordinator of client's care including referral to other services as appropriate.
Supervises LPNs and home health aides.
Participates in in-service programs.
Maintains confidentiality in all aspects of job performance.
Performs other related functions as required. Additional duties as assigned.Minimum Qualifications :
Education:
a) An Associate degree in nursing from a school accredited by the degree in nursing from a school accredited by the National League for Nursing. Baccalaureate degree preferred.
b) Recent, pertinent continuing education.
Experience:
A minimum of one year's home care experience is preferred.

Knowledge, Skills, and Abilities :
Thorough knowledge and skill in current nursing practice.
Knowledge of the concept of team nursing.
Knowledge of the development of plans for client care and the ability to participate in them.
Ability to accept and utilize supervision.
Ability to recognize the needs and concerns of people, and to maintain constructive working relationships in dealing with them.
Ability to supervise.
Ability to communicate effectively both verbally and in writing.
Special Requirements :
Current licensure to practice as a registered nurse in Colorado.
Special certification as appropriate.
Valid, current Colorado driver's license.
Access to a reliable automobile.
