Job Title: Registered Nurse Home Health
Job Description:
We are seeking a compassionate and experienced Registered Nurse to join our team as a Home Health Nurse. The primary responsibility of this role will be to provide high-quality, personalized care to patients in their homes. The ideal candidate will have excellent clinical skills, a strong sense of empathy, and the ability to work independently.
Responsibilities:
- Conduct thorough assessments of patients' health status and create individualized care plans
- Administer medications and treatments as prescribed by physicians
- Monitor patients' progress and update care plans as needed
- Educate patients and their families on health management and self-care techniques
- Coordinate care with other healthcare providers, such as physicians, pharmacists, and physical therapists
- Provide emotional support to patients and their families during challenging times
- Document all care provided and communicate effectively with the healthcare team
Qualifications:
- Valid Registered Nurse license in the state of practice
- Minimum of 1 year of nursing experience, preferably in home health or a related field
- Strong clinical skills and the ability to make independent judgments
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced and ever-changing environment
- BLS certification required, ACLS certification preferred
Working conditions:
This role requires frequent travel to patients' homes, so a valid driver's license and reliable transportation are necessary. The ability to lift and transport equipment and assist patients with mobility is also required.
