What You'll Be Doing:
Coordinate Care: Manage patient care plans, including scheduling appointments, confirming attendance, and assisting with referrals.
Provide Education and Advocacy: Offer educational information and support to parents and caregivers throughout the care process.
Connect Families with Resources: Serve as a point of contact for internal and external community and family services.
Resolve Issues: Assist families with financial, psychosocial, functional, and administrative challenges.
Maintain Accurate Records: Develop and maintain systems to document, track, and monitor patient data.
Provide Specialized Care: Utilize your expertise to provide patient care and resources specific to pediatric diseases and conditions.
Empower Families: Encourage and coach parents and caregivers to actively participate in their child's care.
Collaborate with the Team: Work with a multidisciplinary team to address patient and family needs.
What You'll Bring to the Team:
Experience: 3 years of progressive clinical RN experience, ideally with pediatric patients with Type 1 Diabetes and experience with diabetes technology.
Licensure: Active RN license eligible to practice in Virginia.
Education: Bachelor's Degree in Nursing.
Certifications: Basic Life Support (BLS) upon start; Certified Diabetes Care and Education Specialist (CDCES) preferred.
Schedule: Available to work Monday to Friday, 8 am to 5 pm.
