We're seeking a dedicated Pediatric Nurse Navigator to join our team and provide vital support to diabetic pediatric patients.
Your Opportunity: Be a Guiding Light for Families
As a Pediatric Nurse Navigator, you'll be a liaison, advocate, and point of contact for parents and caregivers, helping them navigate the complexities of pediatric diabetes care. You'll ensure they have access to the best healthcare outcomes and critical resources, providing support and guidance every step of the way.
What You'll Be Doing:
Care Coordination: Coordinate patient care plans, including appointment scheduling, attendance confirmation, transportation, and referral assistance.
Education & Advocacy: Provide educational information and advocacy to parents and caregivers.
Single Point of Contact: Serve as a single point of contact for internal and external community/family services.
Resource Assistance: Assist families with financial, psychosocial, functional, and administrative issues.
Data Management: Develop and maintain systems to document, track, and monitor patient data.
Specialized Care: Utilize your pediatric expertise to provide patient care and resources specific to pediatric diseases and conditions.
Family Empowerment: Educate and coach parents and caregivers to be active participants in their child's care.
Multidisciplinary Collaboration: Collaborate with a multidisciplinary team to address questions and concerns.
Payor Navigation: Discuss payor criteria and resolve issues with payors as needed.

What We're Looking For:
Experience: 3 years of progressive clinical experience (pediatric Type 1 Diabetes preferred).
Licensure: RN license eligible to practice in VA.
Education: Bachelor's Degree in Nursing.
Certification: Basic Life Support (BLS) upon start; Certified Diabetes Care and Education Specialist (CDCES) preferred.
Skills: Experience with diabetes technology (CGMs, insulin pumps).
Work Details:
Schedule: Monday to Friday, 8 am to 5 pm.
Setting: Outpatient.
Location: Fairfax, VA.
