Your Role & Responsibilities
Delivering Patient-Centered Care
Coordinate and deliver compassionate care to patients and their caregivers.
Conduct nursing assessments and develop personalized care plans to meet patient and family needs.
Implement, evaluate, and update care plans to ensure continuity and achievement of care goals.
Educate patients and families about their condition, medications, and symptom management.
Encourage patient and caregiver independence whenever possible.
Follow established patient care protocols, standing orders, and clinical guidelines.
Collaborating with the Interdisciplinary Team (IDT)
Participate in interdisciplinary team meetings to discuss patient care plans and progress.
Maintain effective communication with healthcare providers, team members, and families.
Identify patient needs and coordinate referrals to the appropriate team members.
Documentation & Compliance
Ensure accurate, complete, and timely documentation in accordance with regulatory and organizational standards.
Maintain compliance with infection control practices and universal precautions.
Keep leadership informed of any complex patient or caregiver concerns.
Professional Growth & Accountability
Stay up to date with best practices in hospice and palliative care.
Maintain an active nursing license, CPR certification, and any required CEUs.
Exhibit strong time management skills and the ability to respond effectively to patient needs.
Additional Responsibilities (Inpatient Unit Only)
Maintain cleanliness of patient rooms and common areas.
Ensure supplies are well-stocked and appropriate cleaning protocols are followed.
Qualifications
Education: Diploma or Associate Degree in Nursing required; Bachelor's Degree preferred.
Licensure: Active and unrestricted compact (multistate) RN license required.
Certification: Current CPR certification required; CHPN preferred.
Experience:
Minimum 1 year of experience required for inpatient unit (IPU) nurses.
Minimum 2 years of experience required for other nursing positions.
Experience in direct care of patients with life-limiting illnesses required (hospice, palliative care, geriatrics, oncology, home health, or related fields).
Technical Skills: Proficiency with electronic documentation systems.
