Position: Master Social Worker / Case Manager
General Summary:
This position provides psychosocial evaluation, counseling, and resource referral services to clients. The Master Social Worker collaborates with the multidisciplinary treatment team to incorporate psychosocial needs/concerns into care planning, develops resources to meet client needs, and participates in client/family and staff orientation, education, and in-service activities. Documentation of services is maintained in accordance with facility policies and applicable regulations.
Education:
Master’s degree in social work with a specialization in clinical practice from a school of social work.
Must meet state licensure requirements for master-prepared social workers in clinical practice.

Essential Functions and Responsibilities:
Provides psychosocial assessment and counseling services to clients and their families.
Provides case management, including needs assessment, service coordination, monitoring, evaluation, and advocacy.
Refers clients/families to community services (e.g., healthcare coverage, income maintenance, transportation, vocational rehabilitation).
Collaborates with the interdisciplinary treatment team in planning client care and participates in team meetings.
Develops and/or improves facility and community resources to meet client needs.
Participates in client/family and/or staff orientation, education, rehabilitation, and treatment programs.
Advocates on clients’ behalf within the setting and with local, state, and federal agencies.
Participates in facility staff meetings.
Maintains social work records and statistics and participates in quality improvement activities.
Participates in professional social work organizations and community activities serving clients.
Keeps abreast of research/literature/developments in social work and attends continuing education.
